Blood Tests-Child (Paediatric Clinic)



Important: NOTE TO HEALTHCARE PROFESSIONALS

This routine, service is suitable for ALL paediatric patients, from birth. It is a clinician-led service, with no lower age restriction. Sessions run weekly & are now exclusively booked by a patients practice . All patients MUST have a blood request form on arrival. Failure to provide this will prevent blood collection. Please direct parents/carers/guardians to this website ahead of a visit so as children can be familiarised with the blood testing process (see "Blood Test Process-Videos").

⚠️Update 31st March 2026 - Service suspended⚠️

Unfortunately, the service at Formby Village Surgery in unavailable at present. A temporary service will be operational in early September and we will share details of clinic times and how to book into these next week. In the meantime the following services are available:

ROUTINE blood tests

0 - 12 years:

  • Sefton-wide: this service is offered by Alder Hey - families call 0151 282 4589 to book an appointment – a GP blood request form is required
  • Southport Centre for Health and Wellbeing: Alder Hey offer a community clinic every Wednesday morning (8.30 – 11.15am) - families call 0151 282 4589 to book an appointment – a GP blood request form is required

12 – 14 years – available through Litherland Town Hall (Thursday 3.30 – 7pm) - via Choose and Book - a GP blood request form required

14+ years - available through Merseycare community clinics – via Choose and Book - a GP blood request form is required.

URGENT blood tests (if required within 48 – 72 hours):

0 - 16 years – available at Alder Hey hospital site or Southport Centre for Health and Wellbeing – GP/clinical member call service manager on 0151 228 4811 (bleep 104) to book an appointment – a GP blood request form is required

0 – 12 years – available at the Ormskirk Hospital site (Paediatric Assessment and Observation Unit) – refer to Paediatric Registrar by calling 01695 577 111 – a GP blood request form is required

12+ ages – available at Mersey & West Lancs S&O clinic sites – no appointment required - a GP blood request form required.

INFORMATION FOR REFERRING CLINICIANS:

The Formby Village Surgery Paediatric Phlebotomy Service is now bookable through GP Connect. This service is specifically available for children under the age of 16 years.

Provide patients (from the registered sites below) with a blood form, which a relative must bring to the appointment.

Patient Suitability:

This specialist service is clinically-led and is appropriate for paediatric patients, from birth. The team have extended skills in challenging/complex paediatric access.

AMETOP/EMLA:

This service regularly gives patients and their families the option of using 'freeze spray' (cyrogesic), mitigating the requirement for you to prescribe AMETOP/EMLA prior.
